Job Description:

The Social Services Case Manager provides case management for Corps social services under the direction of the Corps Officers, with professional supervision provided by Divisional Headquarters Human Services staff. The Social Services Case Manager will be able to identify the needs of the clients served and provide outreach within the community at large. Perform intakes, interviews, and assessments of clients' needs and perform follow-up case management with households for other supportive programs available if needed beyond emergency assistance. Identify and interact with case management clients, providing comprehensive case management, including but not limited to assessing client needs, developing a plan of action utilizing a strengths-based approach, and identifying internal and external programs that would support their pathway to self-sufficiency. Network with community providers and advocate on clients' behalf, empowering clients to make the best use of community and mainstream resources. Consult with external agencies to provide wraparound support services and resources. Conduct case conferences with involved service agencies, family and caregivers, and other informal supports. Ensure effective use of resources and maintain established relationships with other organizations. Coordinate and facilitate workshops on financial management, nutrition, medical care, and related topics as needed, encouraging clients to attend at least two (2) workshops throughout the year. Coordinate HOPWA services with other service providers to provide housing assistance in a timely and efficient manner. Verify tenancy with landlords and conduct home visits will all clients monthly from assigned HOPWA caseload. Provide case management/supportive services to members who have formerly received HOPWA assistance to ensure they remain stably housed. Generate monthly case manager program updates. Ensure accurate and timely submission of records and reports in applicable databases for all community programs and funding sources as requested by supervisors. Ensure all client interactions are documented in physical and electronic files. Maintain accurate, confidential, and updated case records following The Salvation Army record-keeping policies and procedures. Ensure compliance with CARE/HOPWA grants and other fund requirements. Represent the Salvation Army at community meetings as assigned, and provide feedback and information helpful to the operation and service delivery of the Corps. Actively participate in case conferences, staff meetings, service coordination meetings, supervision, and professional and in-service training sessions. Assist with special events planning, seasonal assistance, and implementation (i.e., Angel Tree program, Thanksgiving & Christmas assistance, Backpack Distribution, etc.). Other duties as assigned by supervisors. Associate degree required; Bachelor's degree preferred. Minimum of one (1) year of case management required. Bilingual (English/Spanish) required. Knowledge of community resources and counseling/social work practices with high-risk populations. Excellent verbal and written communication skills, with the ability to establish rapport. Proficient in Microsoft Office programs (Word, Excel, Outlook, Teams). Possess strong organizational skills and a professional demeanor. Ability to maintain the confidentiality of interactions with clients and fellow employees. Ability to work independently with a strong focus on task completion and meeting deadlines. Proven ability to work with individuals from diverse socioeconomic, racial, cultural, and religious backgrounds, demonstrating a non-judgmental attitude and clear sense of boundaries. Valid driver's license, clean driving record, and access to a reliable, insured vehicle during work hours; must pass MVR check to drive Salvation Army vehicles. Must understand and support the mission of The Salvation Army.
