Job Description:
AccentCare is seeking a dedicated Registered Nurse (RN) for our
Hospice After Hours team, who will be responsible for providing
end-of-life care to patients during non-business hours. This
position requires exceptional nursing skills, a compassionate
approach, and the ability to work autonomously in a home setting.
Our after-hours RNs ensure that our patients receive continuous,
comfortable, and dignified care during nights, weekends, and
holidays. As an integral part of our hospice team, you will work
closely with patients' families, providing necessary support and
guidance during challenging times, helping uphold AccentCare's
commitment to enhance the lives and well-being of our patients.
Provide direct patient care to hospice patients in their home
setting during assigned after-hours shifts, including nights,
weekends, and holidays. Assess patient conditions during every
visit, ensuring comfort and quality of life are maintained.
Administer medications, perform symptom management and assist with
other forms of therapeutic treatment as ordered by the physician.
Educate and support patients' families on end-of-life care and
procedures. Coordinate and communicate with physicians, hospice
team members, and other healthcare professionals to ensure optimal
care. Document care delivered and the patient's condition and needs
in an accurate and timely manner. Respond to emergency situations
with appropriate interventions and consult with the on-call
physician regarding complex clinical issues. Strong proficiency in
hospice care and symptom management Excellent communication and
interpersonal skills Ability to provide empathetic, effective
patient and family-centered care Highly organized and able to work
under pressure in a home-care setting Medical Insurance Dental
Insurance Vision Insurance 401k Current RN license in the state of
employment Valid driver’s license and reliable transportation CPR
certification Additional certification in hospice or palliative
care nursing (CHPN) preferred
